Academically disqualified students may submit an appeal if they have extenuating circumstances surrounding their disqualification. Appeals must demonstrate exceptional, documented circumstances or errors on part of the university. A successful appeal of disqualification will result in the student remaining on academic probation. Note: disqualification appeals are reviewed on a case-by-case basis.

and include a typed statement and supporting documentation, addressed to the Academic Appeals Board, that discuss the following (if applicable):
- The circumstances that have contributed to your academic performance, including how they affected your schoolwork.
- How circumstances have changed since you were disqualified, and the steps you have taken to resolve the difficulties in your academic or personal life.
- What your academic plan will be upon reinstatement to ensure success (i.e. services you will seek, study methods/actions you will follow, etc.).
